Under the supervision of a pharmacist and direction of the Pharmacy Technician Supervisor, the Pharmacy Medication History Technician serves as a key and integral part of the progressive pharmacy managed medication reconciliation practice model. This role has primary responsibility to accurately obtain and compile current, detailed, and complete home medication history lists and allergies for patients to be admitted to the hospital. The technician will use all relevant sources to obtain the most current required information, including patient and family interviews, and direct contact with outpatient pharmacies, extended care facilities, and provider offices. The role involves accurately documenting detailed and verified current home medication lists, including relevant related information, in the electronic medical record (EMR), and reviewing and updating patient allergy information in the EMR. The technician will interact effectively and compassionately with patients and families, and professionally with other healthcare providers. Participation in continuous quality improvement activities and contributing to a highly effective multidisciplinary workgroup are also key aspects of this position.
