As an Acute Medication History Pharmacy Technician, this role obtains and documents accurate medication histories by interviewing patients, families, and other sources. Responsibilities include collecting complete information on prescription medications, over-the-counter products, vitamins, supplements, and herbal remedies to support safe and effective patient care. The shift for this position is 8-hour shifts, 5 days per week, with every other weekend rotation. Typical shift hours are 10:00 AM–6:30 PM or 3:00 PM–11:30 PM, depending on staffing needs. Hire On Incentive up to $2,500 available. Enjoy a flat rate $1/hour weekend shift differential and an 18%-night shift differential when applicable. POSITION SUMMARY This position, under the supervision of a pharmacist, is responsible for obtaining a patient’s complete medication history through communication with the patient, family, and/or other resources and documenting it into the medical record. Medication History technicians (MHTs) perform an important function in collecting an accurate and complete medication history from every patient they encounter. A complete medication history will include prescription medications, over-the-counter (OTC) products, herbal supplements, vitamins/minerals, and other similar products.
