About The Position

Our inpatient pharmacy team collaborates closely with healthcare providers to ensure optimal medication management for hospitalized patients. We oversee the medication storage, distribution, and management in a variety of settings. We review medication orders for accuracy and potential interactions, accurately prepare and dispense medications, provide evidence-based drug information, monitor drug levels to optimize therapy, and educate patients and families about their medications. By prioritizing medication safety and minimizing adverse events, we strive to improve patient outcomes and contribute to their overall well-being. Corewell Health Farmington Hills opened on Jan. 19, 1965 as a 200-bed facility known then as Botsford General Hospital. Today, Corewell Health Farmington Hills Hospital is a 305-bed teaching facility with Level II trauma status.

Requirements

  • Required High School Diploma or equivalent
  • 1 year of relevant experience Pharmacy Required
  • LIC-Pharmacist Intern - STATE_MI State of Michigan required Or
  • LIC-Pharmacy Technician - STATE_MI State of Michigan required And
  • CRT-Pharmacy Technician, Certified (CPHT) - PTCB Pharmacy Technician Certification Board 90 Days required Or
  • CRT-Exam for the Certification of Pharmacy Technicians (ExCPT) - NHA National Healthcareer Association 90 Days required

Nice To Haves

  • Preferred Associate's Degree health care or related field
  • 2 years of relevant experience Retail Pharmacy Preferred

Responsibilities

  • Provides technical assistance in the documentation and clarification of medication history lists under the direction of the pharmacist.
  • Processes, enters, and interprets physician/nurse medication histories, under the direction of the pharmacist, and completes full history documentation in computerized system.
  • Clarifies information through communication with providers, patients, and pharmacies.
  • Performs special projects for the department.
  • Works with multiple medication automation systems.
  • Provides professional customer service as a liaison between the pharmacy department and patients/caregivers.
  • Interacts with prescriber as needed.
  • Processes physician/nurse/pharmacist obtained patient histories and enters into the computer system and assists in maintaining patient profiles.
  • Reviews profile for allergies, duplication of drugs, and last doses and brings these to the attention of the pharmacist.
  • Checks and verifies accuracy of patient profiles.
