About The Position

Carolina Family Health Centers, Inc. is a nonprofit, federally qualified health center (FQHC) dedicated to providing accessible and affordable healthcare to the community, regardless of an individual's ability to pay or insurance status. Our integrated model offers primary medical care, behavioral health, pharmacy, and dental services. The Pharmacy Technician II - Clinical Services at our Wilson Community Health Center plays a crucial role in supporting comprehensive clinical pharmacy and enabling services. These services include medication therapy management, medication adherence clinic, value-based model appointments, and Medicare Part D counseling. While this position will also assist in the dispensing pharmacy as needed, a significant focus is on patient outreach for clinical services, managing appointment schedules for clinical pharmacists, conducting preliminary patient interviews, and performing data entry for documentation and collection. The role requires a high level of accuracy, effective communication with patients, pharmacists, and other healthcare professionals in an ambulatory clinic/pharmacy setting.

Requirements

  • High school diploma or GED.
  • National Pharmacy Technician Certification through Pharmacy Technician Certification Board is required.
  • Computer literacy.
  • Must be self-directed with good organization skills.
  • Excellent communication skills with a high level of comfort interacting with medical providers, pharmacists, pharmacy department staff, and patients.
  • Previous pharmacy work experience is required.
  • Valid North Carolina Driver's license - travel is required to all Carolina Family Health locations.
  • Within 6 months of hire, must become a trained Medicare counselor through the North Carolina Department of Insurance Seniors' Health Insurance Information Program.

Responsibilities

  • Support comprehensive clinical pharmacy and enabling services, including medication therapy management, medication adherence clinic, and value-based model appointments.
  • Contact patients by phone for various clinical pharmacy and/or enabling services, including appointment scheduling, reconciling medication lists, allergy and medical conditions, and educating patients about services.
  • Identify appropriate patients for services through on-line databases and wait lists.
  • Maintain appointment schedules for clinical pharmacists and ensure sufficient patient scheduling.
  • Perform appointment reminders for patients in advance of their appointments.
  • Manage no-showed patient appointments with attempts to reschedule.
  • Provide support to pharmacists by entering data into computer-based or web-based systems for documentation or data collection.
  • Conduct preliminary patient interviews during appointments prior to the pharmacist.
  • Perform traditional Pharmacy Technician II - Clinical Services responsibilities such as prescription entry, medication preparation, and customer service within the in-house pharmacies as needed.
