Pharmacy Technician Care Manager

About The Position
This role supports personalized, comprehensive patient care by ensuring accurate medication management and adherence. As a Pharmacy Technician – Care Manager, you will work closely with healthcare teams and patients to optimize outcomes and improve overall health.
Requirements
- Current Pharmacy Technician License from a recognized certifying body (e.g., PTCB or NHA).
- Previous experience in a pharmacy setting, ideally with a focus on care management or chronic disease management.
- Strong understanding of pharmacology, medication therapy management (MTM), and patient-centered care practices.
- Familiarity with EHR systems, pharmacy software, and telehealth tools.
- Excellent verbal and written communication skills for patient and provider interactions.
- Exceptional accuracy in documenting patient information and managing medication orders.
- A proactive approach to identifying and resolving medication-related issues.
Responsibilities
- Conduct medication reconciliation and resolve discrepancies with care teams
- Educate patients on medication use, side effects, and adherence strategies
- Monitor medication adherence and follow up with at-risk patients
- Support care transitions, including refills, prior authorizations, and delivery coordination
- Maintain accurate documentation in EHR systems
- Collaborate with pharmacists to identify drug interactions and improve care plans
- Ensure compliance with HIPAA and pharmacy regulations
- Participate in quality improvement initiatives
