About The Position

The Inpatient Pharmacy Department at Valley Children’s utilizes evidence-based therapeutic guidelines to provide exceptional pharmaceutical care to our patients. Our highly trained pharmacists and pharmacy technicians work side-by-side with providers and other interprofessional teams to ensure optimal patient outcomes, while utilizing state-of-the-art technology to maximize patient safety. Inpatient Pharmacy provides clinical interdisciplinary pharmacy services in all inpatient areas including Rehab, PICU, NICU, Acute Care, and Oncology. We have numerous pharmacy advance practice protocols including antibiotic monitoring, anticoagulation, IV to oral conversion, and renal dosing. Other clinically supported services include the Emergency Department, Palliative Care, Investigational Drug oversight, and parenteral nutrition (including production). Our team members also support the following specialty clinics: Oncology, Hematology, Rheumatology, Pulmonology/CF, Endocrinology, Neurology, Cardiology and our complex primary care center. Many pharmacists hold collaborative practice agreements to optimize medication therapy management and collaborate to enhance patient care.

Requirements

  • High School Diploma/G.E.D.
  • CPHT - Certified Pharmacy Tech - PTCB - Pharmacy Technician Certification Board
  • TCH-Pharmacy Technician License - PTCB - Pharmacy Technician Certification Board
  • Minimum three (3) years Hospital pharmacy, medical clinic, or retail pharmacy experience.
  • Computer Skills Proficiency with the Microsoft Office Suite.
  • Demonstrated knowledge of outpatient and retail drugs (prescription and non-prescription) and usual pediatric doses.

Responsibilities

  • Participating in medication reconciliation for patients who have been identified as high risk for medication discrepancies.
  • Obtaining comprehensive medication histories interviews with patients caregivers, as well as communication with outpatient pharmacies and other sources.
  • Accurately documenting and updating the medication list in the electronic health record.
  • Communicating completion of medication reconciliation and identified discrepancies to the patient's care team (physician, nurse and pharmacist).
  • Participating in Quality Assurance/Quality Improvement projects related to medication reconciliation.
