About The Position

Manages and improves the medication-use process through evidence-based, patient-centered medication therapy management with interdisciplinary teams. Provides medication and practice-related education/training and utilizes medical informatics.

Requirements

• Completion of or enrollment in a PharmD program from an accredited college or university.
• Licensed or eligible for licensure as a pharmacist in New York State.
• If not licensed in NYS as a pharmacist must apply for a NYS Pharmacy Intern permit prior to starting the residency and must be licensed within 3 months of being hired.

Nice To Haves

• One year pharmacy experience.
• Leadership experience in academia and/or professional organizations along with above noted education.
