About The Position

The goal of this residency is to enable the resident to develop the skills necessary to provide a high level of pharmaceutical care in a variety of practice settings. Responsible for participating and promoting the clinical pharmacy activities of the Pharmacy. Involvement in daily medical rounds, protocol management, and development of new programs as dictated by the Director of Pharmacy, Clinical Pharmacists, and staff pharmacists. Enhance communication and drug information provisions provided by the Pharmacy to Healthcare workers and patients. Actively participate in the education of pharmacy students. Become competent in management of the Pharmacy distribution system in order to work independently in any position within the Pharmacy. As a fully integrated and active member of the healthcare team, provides optimal drug therapy outcomes in his/her specialized areas. Possesses and utilizes creative skills and sound knowledge in educating other healthcare professionals, patients, students, and the community on specialty related topics. After completing this 12 month residency, the pharmacist will have gained the knowledge and experience to be a competent Specialized Practitioner.

Requirements

  • PharmD degree by an accredited pharmacy school or by the Accreditation Council of Pharmacy Education (ACPE).
  • Current and valid New York State pharmacy license at the time of starting the residency OR Current and valid Out of State license and obtain New York state license by October 1st of the residency year.
  • PGY1 Pharmacy Residency Certificate.
  • Excellent communication (both oral and written), interpersonal, organization, and leadership skills.
  • Good computer skills (Microsoft Office).
  • Qualified candidates must be able to effectively communicate with all levels of the organization.

Responsibilities

  • Performs other duties as assigned.
  • Ascertains and assesses the medication and drug use history of patients and document pertinent findings in the patient's medical record. Completes progress notes, consults, and ensure these notes are reviewed and co-signed by a preceptor.
  • Rounding with medical teams. Monitoring patient medications daily. Performs pharmacokinetic (PK) calculations and investigates adverse drug reactions (ADRs).
  • Assesses patient condition and develops treatment plans daily. Communicates with attending/fellows/residents/interns/NP/PAs regarding changes in treatment plans. Controls medication administration. Assures that medication orders or other data is entered into the patient record or profile.
  • Participates in the discharge planning process to insure that the patient's pharmaceutical needs are met. Performs patient counseling with evidence based medicine.
  • Supervises and directs the work completed by pharmacy technicians and other support personnel.
  • Demonstrates the ability to evaluate and analyzes drug literature and to effectively communicate complex information to a broad audience in both formal and informal presentation settings
  • Demonstrates leadership skills and exhibit a commitment to the profession of pharmacy specialty.
  • Should be adept in problem solving and managing a clinical practice.
