Supports pharmacy services by obtaining medication and allergy histories for patients that will be admitted to the hospital. Information is obtained by interviewing the patient, patient family, calling pharmacies, long term care facilities and/or other providers. Information obtained is communicated to pharmacists and health care providers to determine treatment plans and medication orders during hospitalization and post discharge from the hospital. Obtains an accurate medication and allergy history by interviewing the patient, family, pharmacy, long term care facility, and/or other providers. Screens and reviews patient medications for problems related to duplications, contraindications due to allergies or new conditions, NPO status, and communicates problems to the pharmacist. Accurately creates a medication reconciliation list that contains the drug name, dose, strength, frequency, and route for all prescription, over the counter, herbal, and professional samples that the patient is taking, and validates how the patient takes their medications. Documents changes in the historical medication list within the EMR.
