About The Position

The Medication Reconciliation Pharmacy Technician is responsible for working with newly admitted patients in the ED to obtain and document an accurate home medication list to help avoid medication duplications, omissions, and errors. The incumbent obtains and enters information used by physicians and other healthcare providers to determine treatment plans and medication orders during and post-discharge from the hospital. This position is also responsible for handling the drug distribution system and providing technical support in all aspects of the delivery of safe, effective, patient-focused, and cost-efficient drug therapy, and also to assist the pharmacist in the daily functions of the pharmacy department.

Requirements

  • High School Diploma or GED Required
  • 1 or more years retail or hospital-based pharmacy experience.
  • LPT - Licensed Pharmacy Tech Technician Registration with the Georgia State Board of Pharmacy Required
  • PHAR-C - Certified Pharmacy Tech Required

  • Vocational/Technical Degree in Pharmacy technology or related field Preferred

Responsibilities

  • Assisting pharmacists in receiving, processing, and dispensing of all physicians' orders for medications, IV solutions, and outpatient prescriptions.
  • Transporting drugs and supplies to and from the pharmacy area to ensure delivery of quality care according to established protocols and transport times.
  • Completing training in aseptic technique and performing duties and responsibilities of an IV technician.
  • Answering phones, window, and doors, and loading and unloading the pneumatic tube system.
  • Assisting in pulling out dates from the main pharmacy and automated dispensing cabinets.
  • Assisting in stock replenishment and maintaining inventory.
  • Performing monthly nursing station audits with pharmacists.
  • Obtaining and clarifying patient's home medication list by utilizing Meditech and various drug information resources.
  • Contacting community pharmacies, physician offices, or other sources as appropriate for prescription clarification.
  • Interviewing patients/families and reviewing prescription bottles brought to the Emergency Department by the patient to ensure proper identification of doses and drugs, including prescription medications, over-the-counter drugs, and herbals/supplements.
  • Interacting with and providing medication history and other related patient information to nurses, physicians, pharmacists, and other medical professionals as appropriate.
  • Serving as a liaison between pharmacy and nursing to foster medication teaching the patient in a required timeframe.
  • Complying with Pharmacy and hospital policies and procedures.
  • Resolving and ordering missing medications through the central pharmacy.
  • Helping to facilitate the transfer of medications for patients moving off the unit.
  • Checking medication storage areas for discontinued and expired medications and facilitating return to pharmacy.
  • Crediting medications and returning them to the pharmacy for return to stock.
  • Restocking Automated Dispensing Cabinets.
  • Performing monthly inspections for accrediting body readiness.
  • Volunteering and assisting as needed during staffing emergencies and other occurrences that may create unusual staffing circumstances.
  • Communicating with physicians and nurses professionally and courteously on medication-related issues.
  • Taking ownership of problems by reporting issues to appropriate hospital staff to achieve desired outcomes.
  • Involving pharmacy supervisory personnel when necessary to reach goals.
  • Demonstrating technical skill in using equipment appropriate for the role.
