About The Position

United Health Services (UHS) is seeking a Pharmacy Manager, Ambulatory Care Pharmacy to provide strategic leadership, clinical oversight, and operational management for ambulatory pharmacy services across outpatient clinics and physician practices. This role is responsible for the development, implementation, and evaluation of pharmacist-led patient care programs that optimize medication management, improve patient outcomes, enhance quality performance metrics, and support population health initiatives. The Pharmacy Manager collaborates with physicians, advanced practice providers, nursing leadership, and other interdisciplinary teams to advance patient-centered care, ensure regulatory compliance, drive operational excellence, and align pharmacy services with UHS's organizational goals, growth strategies, and commitment to high-quality healthcare delivery.

Requirements

• Doctor of Pharmacy (Pharm.D.) degree from an accredited college or university.
• Current licensure as a Pharmacist in New York State (NYS Office of the Professions) or eligibility for licensure prior to employment.
• Minimum of five (5) years of progressive clinical pharmacy experience, including significant experience in ambulatory care practice.
• Successful completion of an accredited PGY1 Pharmacy Residency program.
• Demonstrated leadership experience with the ability to develop and manage clinical pharmacy services, lead interdisciplinary teams, and drive quality improvement initiatives.

• Completion of an accredited PGY2 Ambulatory Care Pharmacy Residency program.
• Advanced experience in ambulatory care program development, population health management, and value-based care initiatives.
• Board Certified Ambulatory Care Pharmacist (BCACP) credential.

Responsibilities

• Provide strategic leadership for ambulatory care pharmacy services across outpatient clinics and physician practices.
• Develop, implement, and expand pharmacist-led clinical programs that improve medication management and patient outcomes.
• Establish and maintain collaborative practice agreements (CPAs) with providers to support evidence-based patient care.
• Oversee chronic disease management programs focused on conditions such as diabetes, hypertension, heart failure, anticoagulation, and other high-impact disease states.
• Collaborate with physicians, advanced practice providers, nursing, and clinical leadership to integrate pharmacy services into patient care delivery models.
• Direct the day-to-day operations of ambulatory care pharmacy services, ensuring efficient, patient-centered workflows.
• Manage staffing models, scheduling, workload distribution, and resource allocation to meet operational and clinical objectives.
• Monitor key performance indicators (KPIs), productivity metrics, and service outcomes to drive operational excellence.
• Ensure compliance with all applicable regulatory, accreditation, organizational, and professional standards.
• Develop and manage departmental budgets and operational resources in support of organizational goals.
• Identify and optimize reimbursement opportunities for ambulatory pharmacy services, including value-based care initiatives.
• Evaluate program effectiveness and return on investment (ROI) through financial and clinical outcome measures.
• Support business planning and growth strategies for ambulatory pharmacy services.
• Lead quality improvement initiatives aimed at enhancing patient outcomes, medication safety, and care quality metrics.
• Support population health and value-based care programs through targeted medication management interventions.
• Analyze clinical and operational data to identify opportunities for performance improvement and program expansion.
• Partner with organizational leadership to achieve quality, safety, and regulatory objectives.
• Recruit, develop, mentor, and retain high-performing pharmacy professionals and support staff.
• Foster a culture of collaboration, accountability, innovation, and continuous professional development.
• Conduct performance evaluations, establish development plans, and support succession planning initiatives.
• Promote employee engagement and a patient-centered approach to care delivery.
• Serve as a preceptor and mentor for pharmacy students, residents, and other learners within ambulatory care practice settings.
• Support residency and experiential education programs in collaboration with academic partners.
• Encourage and participate in research, quality improvement projects, presentations, publications, and other scholarly activities.
• Stay current with evolving pharmacy practice standards and contribute to the advancement of ambulatory care pharmacy services throughout the organization.
