About The Position

MSHS seeks an Ambulatory Care Clinical Pharmacy Manager that reports to the Senior Director, Ambulatory Pharmacy. The Clinical Pharmacy Manager will have direct patient care and centralized responsibilities aligned with care delivery goals as dictated jointly by Pharmacy and health system leadership. Manager may develop collaborative drug practice agreements with providers (MDs) at practices. Managers will lead medication management performance improvement initiatives. Managers are expected to manage pharmacy students and residents as requested. Manager will assist with design, implementation and evaluation of all related pharmacy initiatives.

Requirements

  • Doctor of Pharmacy Degree required
  • ASHP-Accredited PGY-1 required; PGY-2 preferred
  • Board certification (BCACP) or obtain BCACP within 2 years of hire, required. Depending on the specific patient population that the ambulatory care clinical pharmacy manager cares for, an alternative BPS specialty certification (BCGP, BCCP, BCTXP, BCPP, etc) may be approved by the Director.
  • 3 years hospital experience or equivalent, with clinical experience/training
  • Registered Pharmacist
  • NYS Board of Pharmacy
  • Continues to meet regulatory, educational, and licensure requirements throughout employment, as set forth by New York State regulations.
  • Achieves annual requirement of 15 continuing education hours (ACPE approved).
  • Must be certified (or certified within 6 months of hire) to practice Collaborative Drug Therapy Management (CDTM) by the New York State Education Department
  • American Heart Association BLS certification, required within 2 months of hire
  • Chronic disease management, care management
  • MTM experience, CDTM experience, pop-health experience

Nice To Haves

  • PGY-2 preferred
  • Immunization certification by the New York State Education Department, preferred within 1 year of hire

Responsibilities

  • Delivery of clinical pharmacy services with a focus on chronic disease management
  • Promote clinically rational drug therapy and sound pharmaceutical care through the development of new (or expansion of existing) pharmacy practice programs, drug therapy policies, and other programs (including Chronic Care Management)
  • Serve as a clinical pharmacy resource for the health-system
  • Work collaboratively with clinical staff to implement and modify medication therapy of individual patients (including those with diabetes, obesity/overweight, heart failure etc.)
  • Implements decisions of and provides support for the Ambulatory Pharmacy & Therapeutics committee by preparing drug evaluations and assisting in formulary maintenance
  • Identification, development, and implementation of new pharmacy strategies and innovation pilot projects across responsible clinics/areas.
  • Provide education, training and work direction to clinic staff and practitioners as required.
  • Serve as a preceptor for the MSHS PGY1 and MSHP PGY2 ambulatory care pharmacy residency programs and students from colleges of pharmacy
  • Provide scholarly activities and presentations at local and national meetings to advocate for and raise awareness of ambulatory care pharmacy roles and responsibilities
  • Develop and scale medication management programs, manage goals of pharmacy integration across MSHS.
  • Support financial, quality and utilization analytics and performance improvement evaluations.
  • Serve as pharmacy representative on behalf of ambulatory pharmacy as required on organizational committees.
  • Supervises pharmacy employees
  • Responsible for/contributes to employee performance evaluations for assigned staff
  • Provides off-hour coverage for the department
  • Interviews candidates and supports in leading orientation activities
