The Pharmacy Clinical Specialist – Internal Medicine/Hematology Oncology serves as a decentralized clinical pharmacist embedded within an inpatient solid tumor oncology service, providing comprehensive pharmacotherapy management for hospitalized patients. While practicing within an oncology environment, this role is primarily focused on internal medicine, with the majority of clinical responsibilities centered on managing acute and chronic medical conditions, optimizing medication therapy, conducting therapeutic drug monitoring, performing medication reconciliation, and supporting multidisciplinary patient care. Caregivers collaborate closely with physicians, advanced practice providers, nurses, and other healthcare professionals to develop evidence-based, cost-effective treatment plans while addressing medication-related issues and promoting patient safety. This position offers a unique opportunity for pharmacists with internal medicine training or experience to expand their expertise in hematology and oncology, gaining exposure to cancer therapies, treatment-related toxicities, and supportive care while maintaining a practice that remains heavily focused on internal medicine and the complex medical needs of patients with cancer.

Requirements

  • PharmD or Bachelor of Science in Pharmacy from an accredited college of pharmacy
  • If hired for a specialty area where specialty residency training is available, then specialty residency is required
  • Current state licensure as a Registered Pharmacist (RPh) OR eligible for reciprocation in the current state within 90 days of hire
  • For individuals required to administer immunizations, current Basic Life Support (BLS) through American Heart Association (AHA) or American Red Cross certification and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire are required.
  • For individuals required to respond to adult or cardiopulmonary arrest events, current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) Certification through American Heart Association (AHA), or certification obtained within one (1) year of hire, is required
  • Completion of a post-graduate year 2 residency (PGY2) or fellowship OR completion of a post-graduate year 1 residency (PGY-1) and two years of experience as a clinical specialist or an equivalent role providing pharmaceutical care and performing pharmaceutical monitoring OR four years of experience as a clinical specialist or an equivalent role providing pharmaceutical care and performing pharmacotherapeutic monitoring

Responsibilities

  • Coordinate and provide pharmaceutical care for patients.
