About The Position

The Pharmacy Clinical Services Supervisor is responsible for obtaining and maintaining pharmacy residency accreditation through ASHP, coordinating accreditation preparation and visits, and assembling/chairing the Residency Advisory Committee (RAC). This role collaborates with the RAC to develop training and competency programs for pharmacists, residents, students, and technicians. The supervisor establishes preceptor development guidelines, participates in resident recruitment and selection, and monitors program outcomes. Additionally, the role involves scholarly activities, strategic planning for residency and clinical pharmacy services, and coordinating training for clinical pharmacists. The supervisor ensures compliance with medical staff bylaws for clinical privileging, develops affirmative action plans, and monitors progress. Key responsibilities include monitoring and evaluating drug therapy, performing pharmaceutical compounding, screening prescriptions, evaluating medication orders for appropriateness, and providing patient-centered services. The role also involves dispensing medications accurately, monitoring outcomes, participating in medication reconciliation and interdisciplinary rounds, and advising healthcare providers on medication-related issues. Routine duties include providing pharmaceutical care and public health services in various community settings, offering individualized patient counseling, assisting in educational programs, and contributing to quality improvement and medication safety efforts. Documentation of clinical interventions, quality assurance, and medication error prevention is crucial. The pharmacist oversees pharmacy technicians, supports continuous improvement, and performs administrative functions such as policy development, performance improvement, and regulatory compliance. Participation in pharmacy audits, controlled substance regulation monitoring, and staff training is expected. The pharmacist may serve on committees as a subject matter expert and must complete required training, maintain licensure, and participate in performance reviews. Other duties as assigned by the supervisor are also part of this role.

Requirements

Completion of an ASHP-accredited post-graduate year 1 (PGY-1) residency and a minimum of three (3) years of relevant pharmacy practice experience; Or Completion of ASHP-accredited PGY-1 and post-graduate year 2 (PGY2) residencies and a minimum of one (1) year of relevant pharmacy practice experience; Or Has a minimum of five (5) years of relevant pharmacy practice experience if there is no completion of an ASHP-accredited residency.
Completion of an accredited college or university with a Bachelor’s degree in Pharmacy or PharmD.
Board certified or eligible for board certification. If board eligible, must obtain board certification within six (6) months of hire date.
Active, unrestricted Pharmacy License in any U.S. State or Territory by completing necessary Board of Pharmacy requirements for the respective state of licensing.
Applicant must have a valid, unrestricted insurable drivers license.

Responsibilities

Obtains and maintains pharmacy residency accreditation through the American Society of Health System Pharmacists (ASHP); coordinates accreditation preparation and visits.
Assembles and chairs the Residency Advisory Committee (RAC) to ensure that department leadership, program leadership, and preceptors are engaged in design and oversight of residency program.
In collaboration with the residency advisory committee, participates in the development of the pharmacy department’s training and competency program for pharmacists, residents, students, and technicians.
Establishes preceptor development and training program guidelines; participates in the recruitment, selection, and orientation of new preceptors.
Evaluates qualifications of pharmacy resident applicants through documented, formal, procedural-based criteria.
Participates in the interview, selection, and onboarding process of residents for the program.
Monitor and evaluate program outcomes, including participant performance, progress and satisfaction.
Continually assess skills and development of preceptors as well as resident progression in the program.
Participates in scholarly activities, professional publications, podium presentations, poster presentations, and practice based research.
Provides strategic planning and needs assessment for growth of residency and clinical pharmacy services.
Coordinates didactic and experimental training for new and experienced clinical pharmacists assigned to pharmacist-led clinics.
Responsible for ensuring compliance with qualifications, standards and requirements set forth in the FDIHB Medical Staff Bylaws as they pertain to clinical privileging of clinical pharmacists.
Develops affirmative action plan for the area supervised, including appropriate objectives and goals.
Monitors and periodically assesses progress.
Monitor, modify, and evaluate drug therapy for optimal therapeutic outcomes; interact with patients to obtain drug history, monitor patients' responses, educate and counsel, and explain information on current drug therapy and potential side effects.
Performs pharmaceutical compounding for sterile and non-sterile preparations, including extemporaneous formulations.
Screen prescriptions for proper interpretation; appropriate dosages; and possible therapeutic, chemical, or physical incompatibility; advise patients and healthcare staff on dosage regimen, side effects, and storage requirements.
Evaluate medication orders for appropriateness based on current clinical guidelines, patient-specific factors (e.g., renal or hepatic function, age, comorbidities, etc.), drug interactions, allergies, and approved formulary options.
Services are patient-centered and align with each individual’s therapeutic needs and goals.
Dispense medications with attention to accuracy, labeling, patient safety, and regulatory compliance.
Monitors and evaluates outcomes by reviewing clinical and laboratory data, assessing efficacy and toxicity, and makes evidence-based recommendations.
Participates in medication reconciliation processes, interdisciplinary team rounds, and collaborative treatment planning.
Advises physicians, nurses, and other healthcare providers on medication-related issues, including selection, dosing, monitoring, and potential adverse effects.
Routine duties shall include providing pharmaceutical care and public health services to individuals eligible for services provided by the FDIHB in homes, schools, clinics, job sites, and other community locations within the assigned Service Unit.
Provides individualized patient counseling to promote safe and effective medication use.
Education includes instructions on medication administration, expected effects, potential side effects, interactions, and any necessary lifestyle or dietary modifications.
Assists in conducting interdisciplinary educational programs in drug therapy, and/or develop and provide education materials to patients.
Assists in quality improvement and medication safety efforts, including adverse drug event tracking, drug utilization reviews, and formulary management.
Provide input into interdisciplinary initiatives and contribute to the development of educational materials for patients and staff.
Documents and provides analysis related to quality assurance, medication error prevention, and medication use evaluations.
Helps ensure that pharmacy operations are aligned with facility policy, accreditation requirements, and professional standards.
Education is delivered in a patient-centered manner that considers health literacy, language preference, and cultural context.
All counseling and clinical interventions are documented thoroughly in the health record.
Additionally, the pharmacist oversees the work of pharmacy technicians, ensuring accuracy in dispensing, compliance with legal requirements, and proper medication storage and inventory procedures.
The pharmacist supports continuous improvement initiatives to enhance the safety and quality of medication use throughout the facility.
Performs administrative functions that support the pharmacy department and its integration within the broader healthcare team.
This may include contributing to the development and implementation of policies and procedures, performance improvement activities, compliance with accrediting standards, and preparation for regulatory inspections.
Participates in pharmacy audits, monitors compliance with controlled substance regulations, and supports staff training and education.
The pharmacist may serve on facility committees or workgroups as a subject matter expert in pharmacotherapy or medication safety.
Completes required training, maintains appropriate licensure and continuing education, and participates in their performance reviews.
Performs other duties as assigned by the supervisor in support of pharmacy operations and the overall mission of the healthcare facility.
