About The Position

Performs and is responsible for the day to day professional activities of the Pharmacy Department including duties involving compounding, admixing sterile products, drug consultations, dispensing, providing drug information, and monitoring patient drug therapy. This position is represented by RCHEA.

Requirements

  • Graduate of an accredited college of pharmacy
  • Current California Pharmacist License
  • Able to interface with hospital staff and public in a courteous and professional manner
  • Demonstrates emotional maturity, self-awareness and consistently performs with integrity and respect
  • Must have tact, good judgment, and self-motivation, be detail oriented, respect and protect the confidentiality of sensitive material
  • Employee must conform with all HIPAA and other confidentiality regulations as required by the job, department, or hospital

Nice To Haves

  • Experience in hospital pharmacy practice preferred
  • Residency training preferred

Responsibilities

  • Conducts and accurately documents in the medical record drug protocols as ordered by physicians including, but not limited to, TPN, Pharmacokinetic, and Heparin.
  • Contacts and intervenes with physicians and/or nurses regarding questions, errors, or irregularities on orders.
  • Maintains and meets expectations on time for all competencies, license, certifications and education requirements as outlined by local administration, Adventist Health (AH), The Joint Commission (TJC), Centers for Medicare and Medicaid Services (CMS), and all other regulatory agencies.
  • Maintains computerized patient record system to monitor medication therapy for safety and efficacy, including but not limited to, medications, med errors, and nursing unit inspections.
  • Maintains daily records of controlled drugs as required by law.
  • Monitors and intervenes on all drug regimens as indicated by quality assurance plan, including, but not limited to, drug-drug interactions, drug allergies, toxic/therapeutic drug levels, and appropriateness indications.
  • Provides clinical drug information to meet the needs of the physicians and nursing staff through direct contact and/or committee participation.
  • Reviews inventory levels to ensure that all outdated drugs, or drugs that have lost their potency, are removed.
  • Supervises support personnel in routine performance of their duties, including but not limited to, IV admixtures, medication dispensation and medication compounding.
  • Willingly performs other duties and innovations as assigned.
