Pharmacist - Inpatient/Outpatient
About The Position
The Department of Veterans Affairs offers healthcare services to eligible military veterans at VA medical centers and outpatient clinics across the country. The Phoenix VA Health Care System proudly serves Veterans in central Arizona at the Carl T. Hayden VA Medical Center and nine community-based outpatient VA clinics.
Requirements
- Graduate from an ACPE-accredited college and have at least a baccalaureate degree in Pharmacy, and/or a Pharm.D. degree.
- Any clean, unrestricted pharmacy license from any state is honored.
- BLS certificate (preferably through the American Heart Association).
Responsibilities
- Dispense drugs prescribed by physicians and other health practitioners and provide patients with information regarding medications and their use.
- Advise physicians and other health practitioners on the selection, dosages, interactions, and side effects of medications.
- Evaluate and provide medication orders for pharmacologic duplications, incompatibilities, side effects, drug-food and drug-drug interactions, and allergies.
- Appropriately address potential therapeutic problems such as inappropriate dosage, incorrect strength, or improper dosing intervals.
- Effectively communicate questionable therapies to providers and recommend reasonable alternatives in accordance with the formulary guidelines established.
- Counsel patients and respond to questions regarding prescription drugs, including possible adverse reactions or interactions.
- Provide advice on durable medical equipment and home health care supplies.
- Demonstrate positive interpersonal skills when interacting with providers, patients, other Medical Center employees, and fellow pharmacy staff members in order to foster an atmosphere of care, helpfulness, and cooperation.
- Ensure that all medication orders are entered and verified in the electronic patient medical record (VistA).
- Provide strict security for all drugs and controlled substances by maintaining accurate records and perpetual inventory control.
