Pharmacist - Inpatient/Outpatient
About The Position
This position involves providing comprehensive pharmaceutical services within the personnel, supply, and equipment capabilities of the facility. The role requires performing a full range of pharmacist procedures, including screening patient electronic health records for medication order appropriateness, verifying prescription accuracy, and counseling patients on proper medication use. The pharmacist will also be responsible for compounding medications, ensuring the accuracy of products prepared by technical staff, and supervising inventory control of all pharmaceuticals, including scheduled controlled substances, while maintaining pharmacy security and records according to regulations. The role may require rotation of on-call duties, with the possibility of returning to the pharmacy during nights, weekends, and holidays for limited services such as drug information, inpatient order processing, IV admixture preparation, and Pyxis replenishment. Services will be provided to patients ranging from neonate to geriatric in both outpatient and inpatient settings.
Requirements
- Bachelors Degree in Pharmacy at minimum.
- Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
- One year of professional pharmacy experience after graduation.
- Current, full, active, and unrestricted license to practice as a pharmacist.
- Must possess the ability to pass a Government background check/security clearance.
- Possess a current AHA or ARC BLS Healthcare Provider Certification.
Responsibilities
- Perform a full range of pharmacist procedures.
- Rotate calls with other pharmacists, with infrequent possibility of needing to return to the pharmacy nights, weekends, and holidays for limited services.
- Provide outpatient and inpatient pharmacy services to patients ranging from neonate to geriatric.
- Screen patient electronic health records for appropriateness of the medication order or refill.
- Verify that patient prescriptions are accurately filled and the directions for use are clear.
- Counsel patients to ensure proper use of the medication.
- Perform thorough review of the patient's electronic health record to verify the appropriateness of therapy, including dose for condition, weight, age, and renal/hepatic function, appropriate medication for condition, monitoring for drug-drug or drug-disease interactions, and verification that day supply matches what is ordered.
- Deliver comprehensive pharmaceutical services within the personnel, supply, and equipment capabilities of the facility.
- Counsel patients regarding the appropriate use of medications.
- Review patient profiles at the time of dispensing in order to closely monitor medication therapy.
- Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age-specific dosage, and other pertinent data are appropriate for individual patients.
- Intervention with providers shall be made for noted discrepancies.
- Compound medications.
- Ensure accuracy of all pharmaceutical products prepared or processed by technical staff.
- Supervise inventory control of all pharmaceuticals in the pharmacy, especially scheduled pharmaceuticals, and ensure pharmacy security.
- Maintain records and security of all scheduled controlled substances according to applicable regulations.
