About The Position

The Pharmacist role at Atlantic Health Morristown Medical Center is a full-time position focused on ensuring appropriate and safe medication therapy for patients. This involves reviewing patient profiles, transcribing and verifying medication orders, dispensing medications accurately, and preparing sterile preparations. The role also includes participating in clinical initiatives, providing drug information, and contributing to performance improvement and customer satisfaction. The Pharmacy Department serves multiple outpatient areas, including infusion centers, retail pharmacies, and health clinics, with full-service retail pharmacies at Morristown and Newton Medical Centers offering convenient prescription services for patients, including those being discharged.

Requirements

  • Proficiency in reviewing patient profiles for appropriate therapy.
  • Ability to accurately and timely review and transcribe medication orders.
  • Skill in dispensing medications, prescriptions, and miscellaneous items.
  • Competence in processing requests via various communication channels.
  • Accuracy in checking unit doses, medications for dispensing machines and floor stock, IVs, batching, patient own medications, repackaging, and extemporaneous preparations.
  • Knowledge of IV equipment, sterile technique, total parenteral nutrition, and safe handling of hazardous drug preparation and precautions.
  • Ability to participate in clinical initiatives and make appropriate recommendations.
  • Skill in providing drug information to healthcare professionals and patients.
  • Ability to handle problems/issues independently.
  • Professional and helpful demeanor towards patients, coworkers, and allied health professionals.
  • Ability to direct and monitor technicians.

Responsibilities

  • Reviews patient profiles to assure appropriate therapy based on patient specific information (age, gender, diagnosis, co-morbidities, laboratory values, allergies, height, weight, BSA, pregnancy and lactation status).
  • Reviews and transcribes all medication orders accurately and timely, verifying therapy, medication, route, dose, and frequency.
  • Makes recommendations for alternate treatments when necessary.
  • Records relevant information and drug waste in EHR.
  • Dispenses medications, prescriptions, and miscellaneous items accurately and promptly.
  • Processes requests via phone, window, fax, and computer professionally and timely.
  • Obtains medications considering economical or alternative options and patient admission status.
  • Checks unit dose, medications for automated dispensing machines and floor stock, IVs, batching, patient own medications, repackaging, and extemporaneous preparations for accuracy.
  • Checks crash cart trays, rapid response boxes, and miscellaneous emergency kits.
  • Prepares, supervises, and ensures aseptic compounding of IVs, including antineoplastic agents, correctly and timely.
  • Reviews levels of batched IV medications, batches, and supplies to avoid waste.
  • Assures batches are checked and put away promptly.
  • Participates in clinical initiatives.
  • Initiates appropriate clinical interventions and clarifications with corresponding documentation.
  • Reviews pertinent information to monitor patients’ therapies and makes appropriate recommendations eliminating duplicate, unwarranted, or inappropriate therapy.
  • Converts non-formulary medication requests to appropriate formulary alternatives.
  • Provides drug information to physicians, nurses, other healthcare professionals, and patients/family members.
  • Answers phones, window, and door promptly and courteously.
  • Handles problems/issues independently.
  • Participates in internal and external performance improvement and cost savings initiatives.
  • Supports and actively participates in departmental and system initiatives.
  • Seeks opportunities to work independently on projects, teams, and committees.
  • Is professional and helpful to patients, coworkers, and allied health professionals.
  • Directs and monitors technicians assisting when necessary to maintain workflow.
