About The Position

The Oncology Pharmacist role at Atlantic Health Morristown Medical Center involves a comprehensive review of patient profiles to ensure appropriate therapy based on individual patient information, including age, gender, diagnosis, co-morbidities, laboratory values, allergies, height, weight, BSA, and pregnancy/lactation status. This role requires accurate and timely review and transcription of all medication orders, including specialized ones, verifying therapy, medication, route, dose, and frequency, with the ability to recommend alternative treatments. Responsibilities include recording relevant information and drug waste in the EHR, dispensing medications, prescriptions, and miscellaneous items accurately and promptly, processing requests professionally via various channels, and obtaining necessary medications while considering patient status and cost-effectiveness. The pharmacist will also check unit doses, medications for automated dispensing machines and floor stock, IVs, batching, patient-own medications, repackaging, and extemporaneous preparations for accuracy. A key function is the preparation, supervision, and aseptic compounding of IVs, including antineoplastic agents, ensuring efficiency, cost-effectiveness, and adherence to sterile technique, total parenteral nutrition, and safe handling of hazardous drugs. The role also entails participating in clinical initiatives, initiating interventions, clarifying therapies, and making recommendations to eliminate duplicate or inappropriate treatments, as well as converting non-formulary requests to formulary alternatives. Additionally, the Oncology Pharmacist will provide drug information to healthcare professionals and patients, demonstrating expertise in drug information resources and research skills.

Requirements

  • Reviews patient profiles to assure appropriate therapy based on patient specific information that includes at least the following: age, gender, diagnosis, co-morbidities, relevant laboratory values, allergies, height, weight and BSA, pregnancy and lactation status.
  • Reviews and transcribes all medication orders including specialized orders based on approved protocols accurately and timely verifying therapy, correct medication, route, dose and frequency.
  • Dispenses medications, prescriptions, miscellaneous items accurately and promptly.
  • Processes requests via phone, window, fax, and computer in a professional and timely manner.
  • Checks unit dose accurately making corrections to patient profiles if necessary.
  • Checks medications before being placed in automated dispensing machines and floor stock to assure accurate fill.
  • Checks IVs, batching, patient own medications, repackaging and extemporaneous preparations for accuracy.
  • Checks crash cart trays, rapid response boxes, and miscellaneous emergency kits.
  • Prepares, supervises and ensures aseptic compounding of IVs including antineoplastic agents correctly and in a timely manner promoting efficiency and cost effectiveness.
  • Demonstrates a strong knowledge base of IV equipment, sterile technique, total parenteral nutrition, and safe handling of hazardous drug preparation and precautions.
  • Initiates appropriate clinical interventions and clarifications with corresponding documentation.
  • Demonstrates compliance with Pharmacy and Therapeutics Committee initiatives.
  • Reviews pertinent information to monitor patients’ therapies and makes appropriate recommendations eliminating duplicate, unwarranted, or inappropriate therapy.
  • Converts non-formulary medication requests to appropriate formulary alternatives whenever possible.
  • Provides drug information to physicians, nurses, other health care professionals, and patients/family members.
  • Demonstrates and shares knowledge of various drug information resources and knows how to research and interpret data and information.

  • Reviews levels of batched IV medications, batches, supplies to avoid waste, documents all required information, and assures batches are checked and put away promptly.
