About The Position

Provides pharmaceutical care to patients across inpatient and outpatient settings, ensuring safe, effective, and evidence-based medicine. Includes reviewing and verifying medication orders, monitoring therapeutic outcomes, and providing clinical recommendations to the healthcare team. Collaborates with other healthcare professionals to optimize patient care, participates in quality improvement and patient safety initiatives, ensures compliance with hospital, departmental, and regulatory policies.

Requirements

• Graduation from an accredited School of Pharmacy with PharmD required
• Current active registration and licensure with the State of New Jersey Board of Pharmacy required

Nice To Haves

• Previous hospital pharmacy experience (2+ years) preferred.

Responsibilities

• Review patient profiles to assure safe and effective medication therapy.
• Dispensing of medications.
• Monitoring and reporting drug safety based on current clinical and institutional guidelines, as well as patient-specific criteria.
• Overseeing technician practice/activities as per regulatory requirements.
• Medication storage and documentation.
• Actively participates in clinical services (e.g., stewardships, cost-effective, patient experience).
• Adheres to safety requirements in the handling and preparation of sterile and non-sterile medications and other hazardous agents (e.g., oncology) according to current guidelines (USP 797, USP 800, ASHP, NIOSH).
• Communicates effectively and appropriately with healthcare providers and caregivers and assures continuity of pharmaceutical care between shifts and among team members.
• Participates in the training, education and mentoring of other pharmacists and technicians.
• Participates in quality, safety and other project work related to medication management.
