About The Position

As a hybrid Kidney/Pancreas Transplant Pharmacist at Cedars-Sinai Medical Center in Los Angeles, California, you will maintain the quality and support of highly immunocompromised patients requiring a high level of specialized services. The Kidney/Pancreas Transplant Pharmacist will help manage nephrology consults, pre-transplant infusions for highly sensitized patients, post-transplant infusion patients, and cover in-patient services including managing non-formulary requests. The Pharmacist will be responsible for entering therapy plans, conducting chart reviews, composing letters of medical necessity, as well as participating in peer-to-peer reviews for denied insurance claims. The Pharmacist will also participate in in-patient rounding, which would include discharge teachings, medication reconciliation, ordering, and coordinating last-minute medication changes prior to discharge.

Requirements
