Requirements

  • Pharmacist credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required.
  • Bachelor's or advanced degree in Pharmacy required.

Nice To Haves

  • This PRN pharmacist position will support the Comprehensive Care Clinic at Alexian Brothers Medical Center (Elk Grove Village) and the Pharmacotherapy Clinic at Saint Alexius Medical Center (Hoffman Estates).
  • The Comprehensive Care Clinic operates 2 days per week, coordinating care for older patients managing multiple medications and conditions.
  • The multispecialty care team includes a board-certified doctor specializing in family medicine and geriatrics, a case manager/social worker, a clinical pharmacist and a dedicated medical assistant.
  • The Pharmacotherapy Clinic is a pharmacist-managed clinic specializing in chronic disease management.
  • Active licensure in good standing with the Illinois Department of Financial and Professional Regulation (IDFPR)
  • Completion of an American Society of Health System Pharmacists (ASHP) accredited PGY-1 Residency and ≥ 1 years of experience in ambulatory care practice OR Completion of an ASHP accredited PGY-2 Residency in one of the following specialties: Ambulatory Care, Pharmacotherapy, Family Medicine, Cardiology, Internal Medicine, Geriatric, Nephrology, or Transitions of Care OR Doctorate of Pharmacy (PharmD) or Bachelor of Pharmacy (B.S. Pharm) with at least five years of ambulatory care experience in chronic disease management
  • Clinical pharmacists are preferred to hold nationally recognized certifications (e.g. BCACP, BCPS, BCGP, BC-ADM, CDCES)

Responsibilities

  • Ensure safe and accurate medication management in specialized areas (nuclear medicine, oncology, cardiology) by reviewing, compounding, and dispensing orders, while serving as a subject matter expert and guiding clinical leadership across pharmacy sites.
  • Assess medication orders to identify, prevent, and resolve drug-related problems; recommend formulary alternatives and collaborate with physicians and clinical staff to optimize patient care.
  • Supervise and support technical personnel in medication preparation, maintenance, and dispensing, ensuring compliance with quality and regulatory standards.
  • Provide drug information, patient counseling, and education to medical staff and patients; support academic initiatives as a preceptor and provide operational coverage across pharmacy areas as needed.
