Requirements

  • Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy with at least 3 years of experience
  • Registered Pharmacist (RPh) | Illinois Department of Financial and Professional Regulation (IDFPR)
  • Strong working knowledge of all aspects of clinical guidelines for anticoagulation

  • Anticoagulation management experience is preferred

Responsibilities

  • Manage warfarin including implementation, monitoring, and peri-procedural bridging protocols.
  • Provide high-level Medication Therapy Management (MTM) and patient care to optimize prescribing, administration, and clinical monitoring.
  • Facilitate seamless transitions of care, coordinating hospital discharges with inpatient teams and providing pre- and post-procedure guidance.
  • Manage high-volume INR reviews and patient encounters; triage abnormal lab results and assess clinical status to determine next steps.
  • Counsel patients on therapy adherence, safety considerations, and dosing instructions to improve health outcomes.
  • Directly supervise Anticoagulation Management Service (AMS) technicians and support staff to ensure operational excellence.
  • Collaborate with multidisciplinary teams to develop clinical guidelines, order sets, and evidence-based drug therapy protocols.
  • Ensure all pharmacy operations align with the Illinois Pharmacy Practice Act and relevant government/commercial regulations.
  • Screen for drug-drug, drug-diet, and drug-disease state interactions; provide patient-specific dosing recommendations based on renal function, age, and clinical status.
  • Serve as a pharmacy representative on various hospital committees and workgroups to drive departmental initiatives.
  • Lead the training and orientation of new employees and pharmacy staff.
  • Oversee patient communication channels, ensuring timely responses to inquiries, triage needs, and education requests.
