This position involves demonstrating consistent application of knowledge and skills to deliver appropriate, safe, efficient, and cost-effective pharmaceutical care within the scope of their assigned practice. The Clinical Pharmacist is responsible for the care of patients as well as the development of effective, professional relationships within the multidisciplinary team. This role is specifically for the Virtual Anticoagulation Specialty Clinic (VASC) Service, where the pharmacist will manage anticoagulation therapy under a collaborative practice agreement for patients in Colorado and Montana. Key aspects include collaborating with the healthcare team to ensure safe, high-quality care and effective medication monitoring, acting as a clinical resource to support safe anticoagulation practices, providing drug information to patients and medical providers, and addressing and resolving medication-related problems. The position is primarily remote with occasional requirements for time spent on-site in clinic.
