Join Anticoagulation Management Services, a dynamic team of nurses and pharmacists working in collaboration with referring providers to coordinate and manage anticoagulation care and medication management. The Pharmacist will be responsible for providing pharmaceutical care services for the outpatient Anticoagulation Clinic (AC). This role involves working collaboratively with pharmacy staff, nursing staff, medical staff, and other healthcare providers to assess medication use systems, identify opportunities to improve anticoagulation use safety, and institute practice changes. The position requires management of AC patients through follow-up monitoring and interventions to ensure safety and effective outcomes. The Pharmacist will provide pharmaceutical care following established standards and practices consistent with Sanford policy, regulating bodies (TJC, CMS, etc.), professional practice standards, and state and federal law. Close collaboration with AC nurses is essential for delivering safe and effective care. The role requires visibility and accessibility to clinical staff in various medical and clinical departments. The individual must understand the role of systematic practices in patient care and be able to implement these practices in multiple settings. Cooperation at all levels, both inside and outside the AC, is expected. The ability to understand and disseminate drug information, develop procedures and plans, and supervise students and residents is necessary. Excellent written and verbal communication skills are required, along with knowledge of and utilization of appropriate age-related patient information for pharmaceutical needs of all age groups.
