About The Position

The Personal Care Worker provides essential personal care services and health support to participants, helping them maintain independence and supporting their daily needs. The role includes work in both participants’ homes and a facility, with some travel within the region potentially required. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., and every other weekend. A $2,500 sign-on bonus is offered, paid in three installments in exchange for a two-year commitment. Mercy LIFE of Chester County, part of Trinity Health PACE, provides services to seniors through an interdisciplinary team, supporting them to remain independent at home. Requirements

High school diploma or equivalent.
At least one year of documented experience working with a frail or elderly population; applicants with less than one year must receive appropriate training from the PACE organization upon hiring.
Strong communication and organizational skills.
Valid driver’s license and insurance, and access to a personal vehicle for home care assignments.
Ability to work independently and adapt to change.
Some travel within the region may be required.

Nice To Haves

Responsibilities

Conduct ongoing participant assessments and monitoring.
Assist participants with daily activities, including toileting, grooming, and meals.
Help participants maintain physical activity and exercise routines.
Identify safety hazards and respond to emergencies.
Assist with recreational activities, meal service, and housekeeping.
Report changes in a participant’s condition to center staff.
Escort participants to appointments and assist with transportation.
