About The Position

Rehoboth Elderly Care is a New York State licensed home care services agency serving Nassau, Suffolk, and Queens. It helps older adults and people with disabilities live safely and comfortably at home. The Personal Care Aide lives in a client's home during shifts and provides daily personal care, household assistance, and companionship under a plan of care established by a registered nurse. The position offers a path to become a certified Home Health Aide.

Requirements

Complete NYS DOH-approved Personal Care Aide training or hold a current HHA certificate.
Be listed on the NYS Home Care Registry.
Have a clean NYS DOH criminal history record check; fingerprinting is arranged by the employer.
Obtain medical clearance, including a physical, TB/PPD test, and proof of measles and rubella immunity.
Receive the annual flu vaccine or wear a mask during flu season, as required by NYS rules.
Have a smartphone for electronic visit verification (EVV).

Nice To Haves

Live-in or dementia care experience.
Spanish, Creole, or other language skills.

Responsibilities

Help clients with bathing, grooming, dressing, toileting, and incontinence care.
Assist with walking, transfers, and positioning, using lifts when trained.
Remind clients to take medications; medication administration is not included.
Plan and prepare meals, including simple modified diets, and help with feeding.
Perform light housekeeping and laundry; change linens; shop and run errands.
Escort clients to appointments and community activities.
Provide companionship and promptly report changes in a client's condition to the RN.
Accurately document care, sleep, and meal breaks in the EVV app each shift.
Follow infection control, safety, and HIPAA privacy rules.
