About The Position

Mosaic is seeking a high-energy, dedicated Habilitative Coordinator-QIDP/ Case Manager who brings heart, critical thinking, and a passion for advocacy to their team. This role involves empowering people to lead fulfilling lives by overseeing a caseload, coordinating assessments and meetings, collaborating with stakeholders, training staff, and maintaining documentation. The position is full-time, salaried, with flexible day hours (Monday-Friday) to meet department/agency needs.

Requirements

• Bachelor’s Degree in Psychology, Special Education, or a closely related field
• At least 3 years of hands-on experience working with individuals with intellectual/developmental disabilities.
• Deep knowledge of habilitative services, Intermediate Care Facilities state regulations, and behavior intervention techniques.
• QIDP certification (or state requirements), Med Aide 40, CPR/First Aid and Mandt certification required (if you do not have then required to obtain after hire).
• Exceptional critical thinking, strong problem-solving ability, excellent written communication, and structured time-management skills.

Responsibilities

• Oversee a diverse caseload of up to 22 individuals, creating dynamic, individual-centered programs that support personal growth and independence.
• Coordinate and present key assessments (CFA, POM, SIS), lead annual review meetings, and present before the Human Rights Committee.
• Build strong partnerships with Home Supervisors, families, guardians, and Service Coordinators to seamlessly drive admission dates and maintain regulatory compliance.
• Train direct care staff in behavior modification techniques and help develop innovative service methodologies.
• Monitor the quality of care provided and maintain clear, professional documentation.
