Community Health Partners is seeking a full‑time Advanced Practice Provider (APPs) to join the Perioperative Assessment Clinic team. In this role, the APP will provide comprehensive perioperative care for elective surgeries and endoscopic procedures by collaborating with an interdisciplinary care team. Responsibilities include conducting detailed clinical assessments, developing and managing treatment plans, performing medical diagnoses and prescribing appropriate therapies and treatment plans. The APP will support patients throughout the surgical continuum, including pre-operative assessment and optimization as well as perioperative and postoperative care recommendations, ensuring the highest standards of safety and quality.
