About The Position

The Perinatal Nurse Coordinator is a member of the Maternal/Child Women's Service line who serves as the primary coordinator for complex high-risk maternal patients and their designated significant others, ensuring a positive patient experience by planning, implementing, and collaborating in a multidisciplinary environment for the plan of care. Additionally, the Perinatal Coordinator collaborates with the Manager of Women's services to create and sustain birth and family education programs to enhance the patient experience while maintaining the quality of care.

Requirements

  • Bachelor’s degree in Nursing
  • Current State of Michigan Registered Nursing License.
  • CPR or BCLS Certified.
  • Three years of experience in Perinatal, Obstetrical and/or Neonatal nursing.

  • Masters Degree in Nursing
  • Certified Nurse in subspecialty
  • Experience inpatient, outpatient and family education.

Responsibilities

  • Participates/conducts multidisciplinary perinatal patient conferences for planning and coordination of services for identified high-risk pregnancies through the community offices.
  • Uses Evidence Based Practice to plan, implement and evaluate outcomes for identified high-risk maternal patients and continues to advocate with mother through discharge process.
  • Collaborates with multidisciplinary team members in performance improvement activities including assessment, planning, implementation and evaluation of clinical outcomes and processes for the complex high-risk maternal patient.
  • Communicates the plan of care (perinatal and postnatal) with the patient, family and other health team members both verbally and in writing.
  • Monitors and modifies the plan of care through an interdisciplinary and collaborative process in conjunction with all parts of the health care team.
  • Serves effectively as a patient and family advocate.
  • Promotes on-going team conferences for patients.
  • Supports patient during difficult decision-making periods.
