Per Diem Pharmacist - Sunnyview Hospital
About The Position
The Per Diem Pharmacist at Sunnyview Rehabilitative Hospital manages patients’ daily medication needs and evaluates, recommends, and monitors medication-related therapy. The pharmacist recommends and evaluates criteria for medication-use evaluations and performs those evaluations to determine the therapeutic role of medications across the St. Peter’s Health Partners Continuum. Evaluation findings are used to improve the quality of medication therapy and reduce healthcare costs. The pharmacist participates in the St. Peter’s Department of Pharmacy Quality Improvement Program and special projects, educates patients and healthcare providers about the safe, effective, and economical use of medications, and mentors pharmacy staff. The role includes active bedside care and interactions with patients, families, nurses, and physicians on all units.
Responsibilities
- Manage daily medication needs of patients at Sunnyview Rehabilitation Hospital.
- Evaluate, recommend, and monitor medication-related therapy.
- Recommend and evaluate criteria for medication-use evaluations and perform evaluations to determine the therapeutic role of medications within the St. Peter’s Health Partners Continuum.
- Use evaluation findings to improve medication-therapy quality and reduce healthcare costs.
- Participate in the St. Peter’s Department of Pharmacy Quality Improvement Program and special projects.
- Educate patients and healthcare providers about the safe, effective, and economical use of medications.
- Mentor pharmacy staff.
- Provide active bedside patient care and interact with patients, families, nurses, and physicians on all units.
- Perform admission, transition-of-care, and discharge medication reconciliation.
- Review and enter medication orders.
- Select and recommend therapeutic regimens.
- Provide drug information.
- Precept APPE students.
- Conduct patient-care improvement projects and medication-use evaluations.
- Perform other duties as assigned.
