About The Position

Trillium Health Resources is seeking an In Reach Coach to join their team. This role is responsible for Diversion and In Reach activities for individuals with Serious and Persistent Mental Illness (SPMI) in Adult Care Homes, State Hospitals, Correctional Institutions, and community settings. The In Reach Coach will educate individuals about community resources, services, and supports, including permanent supportive housing. The goal is to ensure members receive whole person care, addressing physical health, behavioral health, and social determinants of health to achieve positive outcomes and facilitate community integration.

Requirements

  • High School Diploma or GED required.
  • One year of lived experience with a behavioral health condition and/or substance use (defined as living in recovery with mental illness and/or substance use disorder).
  • North Carolina Peer Support Specialist Certification
  • Must have a valid driver’s license.
  • Must reside within Trillium’s Mid State Region, which includes the following counties: Anson, Guilford, Montgomery, Randolph, and Richmond.
  • Must be able to travel within catchment as required.

Responsibilities

  • Engage members who meet criteria for TCL special needs population to provide education about permanent supportive housing, supports, services, and other community resources.
  • Partner with Diversion staff, Transition Coordinators, and Next Step Rehousing Team to facilitate transition planning and progression through the TCL process for those members who desire permanent supportive housing in the community.
  • Collaborate with Trillium Integrated Care Nurse, OT, COTA and Clinician to identify complex care needs and clinical service linkage needs.
  • Facilitate community integration, link to services and provide education and support for those members who choose not to transition to permanent supportive housing in the community.
  • Utilize person-centered planning methods/strategies to gather information, to get to know the members and legally responsible persons to adequately address specific needs of the individual.
  • Provide education and support to members and guardians in learning about and exercising rights and responsibilities.
