The Polaris Peer Supervisor is responsible for oversight of the Polaris Transition Team’s programming, supervision of staff, assurance of quality service and collaborative relationships with all stakeholders, including, but not limited to: Washington County Forensic Team, Oregon Health Authority, Oregon State Hospital, and all community agencies. The Peer Supervisor will oversee additional pilot programming expansions of the existing contract and ensure all required documentation and invoicing is completed in accordance with contractual requirements. The Peer Supervisor will report to the Director of Peer Support Services and be a member of the Polaris Transition Team (PTT). PTT will provide wraparound services to individuals who have been referred to New Narrative by the Washington County Forensic Mental Health Team and other community partners with the aim of assisting the referred individuals in accessing transitional housing, connecting to community based mental health or substance use treatment services, and resource gathering (food boxes, medical needs, etc.) to promote overall stabilization while navigating the legal system. The PTT will work with individuals as they are referred by Washington County who may be experiencing co-occurring disorders, and support individuals as they transition into becoming their own best advocate.
