About The Position

The Pediatric RN Case Manager provides skilled nursing care to pediatric hospice patients in their homes and coordinates all aspects of their care. Working with an interdisciplinary team, physicians, and community partners, the role supports compassionate, dignified, personalized care for children and their families. The position is field-based, includes local travel and an assigned PRN on-call rotation, and reports to the Managing Director - Hospice.

Requirements

• Graduate of an accredited Diploma, ADN, or BSN Nursing program.
• Current Texas Registered Nurse (RN) license.
• Minimum one year of pediatric RN experience in a clinical setting.
• Current CPR certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Current TB testing and Hepatitis consent/declination documentation.
• Ability to pass a criminal background check.

Nice To Haves

• Hospice, Critical Care, or Emergency Room experience.

Responsibilities

• Conduct comprehensive physical, psychosocial, and environmental patient assessments.
• Develop, implement, and document individualized plans of care.
• Communicate effectively with patients, families, physicians, and the care team.
• Provide clinical guidance to Hospice Aides and LVNs and assist with training as needed.
• Complete pediatric admissions and prioritize patient scheduling.
• Participate in assigned on-call rotations for emergency patient support.
• Maintain accurate documentation and ensure compliance with infection control and agency policies.
• Support ongoing quality improvement and professional development.
• Perform other duties as assigned.
