About The Position

This position serves as an integral member of the interdisciplinary Inpatient Rehabilitation Team through proactive planning, organizing, coordinating, monitoring, and evaluating services provided to patients admitted for inpatient rehab. Responsible for ensuring consistency and quality of the referral/admission process, direct patient/family pre-admission assessment, coordination of patient admission to inpatient rehab, team collaboration to ensure a safe discharge, referral source marketing and community liaison. Responsible for utilization management. This role is key in communicating plan of care to payors and facilitating prior authorization, payor updates and reimbursement.

Requirements

• Bachelor’s degree in nursing from an accredited school of nursing
• Minimum of five years of applied Case Management experience as an RN/Case Manager
• Current South Carolina licensure as a registered nurse or compact state required
• Certifications in Case Management (ACM, CCM) required within 6 months of hire

Nice To Haves

• Master’s degree is preferred
• Work experience in an inpatient rehab role is preferred
