About The Position

Imagine Pediatrics is a tech-enabled, pediatrician-led medical group dedicated to reimagining care for children with special healthcare needs. They provide 24/7 virtual-first and in-home medical, behavioral, and social care, collaborating with families, providers, and health plans to improve access to quality care. The role enhances existing care teams by offering an additional layer of support with compassion and creativity, focusing on children with medical complexity. The primary location for this position is remote, requiring residency in Tennessee, Missouri, Louisiana, or Illinois, with a required CST time zone. The expected schedule is 3x12s (Monday-Wednesday or Wednesday-Friday) from 7:00 am to 7:00 pm CST.

Requirements

  • Licensed RN in at least one state with eligibility to register for other state licensures.
  • Bachelors in nursing from an accredited university required.
  • 5+ years of pediatrics experience required.

Nice To Haves

  • Outpatient (primary care and/or subspecialty), home health, complex care, pediatric ICU, emergency medicine, etc. strongly preferred.
  • 1+ years' experience in care coordination or case management experience preferred.
  • Bilingual Spanish preferred.
  • Familiarity with Medicaid regulations and services a plus.
  • Value Based Care (VBC) experience a plus.
  • Virtual care experience a plus.

Responsibilities

  • Provide professional and friendly proactive care and triage for clinical issues.
  • Embed a family-centered care philosophy in care delivery.
  • Demonstrate cultural competence and sensitivity, working with culturally diverse populations and seeking additional resources when needed.
  • Manage transition of care for ED/IP/UC care coordination with clinical providers following discharge.
  • Perform comprehensive assessments of a patient’s clinical, psychosocial, discharge planning, and financial needs, establishing clinical milestones and goals.
  • Establish rapport and relationships with patients and families to understand their needs and expectations, assisting them in setting realistic and mutual goals, and integrating cultural awareness into the patient/family interview process.
  • Establish a comprehensive plan of care, in conjunction with the physician, patient, and interdisciplinary team, to address clinical milestones.
  • Communicate the plan of care, including changes and issues, to the patient/family, physicians, and other healthcare team members.
  • Gather sufficient information from all relevant sources to determine the effectiveness of the plan of care, ensuring it is accurate, safe, timely, and cost-effective.
  • Document all care management assessments and interventions.
  • Refer to Social Worker or Behavioral Health for complex psychosocial and discharge planning issues (per criteria) and ensure appropriate follow-up.
  • Consult with other members of the interdisciplinary team (dietary, pharmacy, etc.) to provide safe discharge as appropriate.
  • Perform other duties as assigned.
