Requirements

Active Connecticut Registered Nurse (RN) License
Current BLS Certification
Strong clinical assessment and critical-thinking skills
Excellent communication and interpersonal skills
Ability to work independently in a home care environment
Reliable transportation and willingness to travel throughout Fairfield County

Nice To Haves

Pediatric nursing experience
Tracheostomy and ventilator experience
PALS Certification

Responsibilities

Provide skilled, individualized nursing care in the home setting
Administer medications and treatments as prescribed
Monitor patient conditions and report changes appropriately
Manage and care for specialized equipment, including: G-Tubes, Tracheostomies, Ventilators
Collaborate with physicians, caregivers, and interdisciplinary teams
Educate families on care plans, treatments, and health management
Maintain accurate and timely clinical documentation
Promote patient comfort, safety, and quality of life
