About The Position

Vanderbilt University Medical Center (VUMC) is seeking a compassionate and skilled Pediatric Home Care Occupational Therapist (OT) to join their growing Pediatric Home Health team serving Williamson and Davidson Counties. This full-time 0.8 FTE role offers a flexible 4-day work week, allowing you to make a meaningful impact in the lives of children and families while maintaining excellent work-life balance. As a Pediatric Home Health OT, you will provide individualized, evidence-based occupational therapy services to children from birth through 18 years old in the comfort of their homes. Requirements

  • Licensed Occupational Therapist in the state of Tennessee
  • 1 year of pediatric experience working with patients ages birth to 18 years
  • CPR certification
  • Valid driver’s license and reliable transportation
  • Strong communication, organization, and clinical documentation skills
  • Bachelor's degree

Responsibilities

  • Provide skilled occupational therapy services to pediatric patients in their homes
  • Evaluate developmental, sensory, motor, and functional needs
  • Develop individualized treatment plans tailored to each child and family
  • Educate caregivers and families on therapeutic strategies and home programs
  • Collaborate with interdisciplinary healthcare teams to coordinate care
  • Document evaluations, treatment plans, progress notes, and outcomes in EMR systems
  • Participate in case conferences and care coordination meetings
  • Support medically fragile infants, children, and adolescents with complex needs
  • Formulate functional treatment plan that is goal focused, individualized, safe and age-specific to address deficits and patient/caregiver areas of concern.
  • Evaluate patient/caregiver responses and modify plan of care according to VHCS and VUMC standards.
  • Administer and score standardized and criterion referenced tests.
  • Interpret the results of the assessment and their significance to the patient, family, referring physician and other members of the health care team.
  • Evaluate and incorporate new research findings into the assessment and treatment of patients.
  • Assist with establishing or modifying existing "Standards of Practice" for specific patient groups.
  • Complete documentation according to agency policy.
  • Meet productivity standards.
  • Demonstrate accountability as an individual and team member in meeting patient/family needs
  • Participate in identifying cost effective measures for clinical care.
  • Manage clinical assignment in an organized, efficient manner using human and other resources.
  • Provide therapeutic interventions for identified objectives.
  • Assure coordination of care for all patients including multi-disciplinary coordination as a case manager.
  • Identify and recommend for referral to other services as indicated.
  • Assure patient safety.
  • Establish relationship of trust and confidentiality with patient/caregivers.
  • Practice Infection Control
  • Meet obligation for reimbursement of the services.
  • Incorporate new information gained, from ongoing dialogue with patient, family, team, physician and payer into treatment plan.
  • Provide education/consultation to patient/caregiver/team/community.
  • Assess patient/caregiver learning needs, coordinate necessary teaching, and evaluate teaching outcomes.
  • Effectively identify and use appropriate educational tools and materials.
  • Plan educational programs to assist team/department with perceived need.
  • Identify problems and/or actual or potential variances from goals and communicate with the health care team.
  • Initiate collaboration with the health care team to assure timely identification of the root cause(s) of problems/variances.
  • Participate in finding solutions for problems/variances and implement actions that move patients/caregivers toward desired goals.
  • Adapt communication to needs/level of understanding of audience
  • Clearly and efficiently communicate significant information across the continuum of care and/or from one caregiver to another.
  • Independently collaborate with peers, members of HCT, and physician.
  • Seek leadership's guidance in conflict resolution.
  • Develop and achieve personal and professional goals.
  • Participate in quality improvement activities.
  • Identify need for professional growth and seeks appropriate professional development opportunities, obtaining a minimal number of educational activities annually that meet licensure requirements.
  • Function as an integral member of the team, demonstrating ability to: respect roles and professional boundaries of self and others; follow through with assigned roles and required activities; understand/comply with policies and procedures.
