About The Position

The Patient Transportation Representative is a field-based role primarily operating in the field, traveling to client sites or designated locations, with occasional office attendance for meetings or training. This position may require up to 100% travel within designated regions in Puerto Rico. The ideal candidate will live within the assigned region. MMM Holdings, LLC, operating under Elevance Health, Inc., provides Medicare Advantage and Medicaid plans in Puerto Rico. Elevance Health is a leading health company dedicated to improving the quality of life of communities in the United States, serving over 118 million people through its affiliated companies. The role involves providing transportation services for members, driving them to and from health service centers, confirming coordinated services with affiliates, contacting members for pickup, and following up on services to ensure effective monitoring and compliance. Associates are expected to work independently, be punctual, detail-oriented, empathetic, and possess strong communication and customer service skills. This role requires driving long distances more than 3 days a week.

Requirements

  • Requires Authorization for Medical Care certification issued by the Bureau of Transportation and Other Public Services.
  • Category 4 Driver's License (Chauffer’s license) in good standing.
  • Certificate of Law 300.
  • Current National CPR Foundation Cardiopulmonary Resuscitation (CPR) certification or obtained within 15 days of hire.

Nice To Haves

  • High school diploma/GED preferred.
  • Associate degree in an area related to health is preferred.
  • 1 year of related customer service experience is strongly preferred.
  • Experience working with geriatric population is preferred.
  • Experience handling electronic equipment such as mobile phones and applications are preferred.

Responsibilities

  • Drives members/patients back and forth to their health service centers.
  • Contacts the affiliate to confirm the coordinated service.
  • Contacts the member to report they are near the pickup location.
  • Follows up on the service provided, making changes in the status of services in the platform, to ensure effective monitoring and compliance, and guarantee quality service.
  • Drive long distances more than 3 days a week.
