About The Position

This role is responsible for coordinating patient services within Home Care & Hospice. The Patient Services Coordinator ensures timely scheduling of patient assessment visits and manages clinician schedules to maximize efficiency and productivity. This position involves direct communication with patients, field staff, and clinical managers to address scheduling issues, ensure all ordered services are included, and facilitate smooth patient transitions. The coordinator also plays a role in training new hires and maintaining a high level of cognitive function and concentration throughout their shift.

Requirements

  • High School Diploma, GED or higher level of education.

  • Knowledge of legal aspects and liability of nursing practice.
  • Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience.
  • Act 34-PA Criminal Record Check from the PA State Police system.

Nice To Haves

  • Associates or Secondary diploma program in a secretarial coursework.
  • 1-2 years secretarial experience in a healthcare environment.
  • Experience in Healthcare setting.

Responsibilities

  • Ensure patient assessment visits are scheduled and performed timely.
  • Prepare weekly clinician schedules and review them daily to ensure productivity standards are met.
  • Assign and schedule clinicians efficiently by geography, matching skills with required care, and optimizing capacity.
  • Ensure missed visits are scheduled promptly or referred to the Clinical Manager.
  • Collaborate with field clinicians to ensure all ordered patient services are scheduled and completed.
  • Manage calls from patients and field staff regarding scheduling issues.
  • Refer clinical and performance-related issues to the Clinical Manager.
  • Complete requested schedules, ensuring all orders and services (e.g., lab draws, supervisory visits, add-on visits) are included.
  • Support Clinical Managers in communicating with patients, clinicians, and office personnel.
  • Run applicable reports and take appropriate actions based on workflow.
  • Assist with the internal transfer of patients between Home Health and Hospice for scheduling.
  • Make and communicate all scheduling changes and updates within the scheduling system to affected staff.
  • Extract referrals from software and follow scheduling assignment guidelines.
  • Print various reports for supervisors (e.g., expiring cert list, active patients, case manager).
  • Work closely with Supervisors on staffing and scheduling issues.
  • Consult with supervisors if patients need to be moved between staff members.
  • Ensure messages reach the appropriate individuals in a timely manner.
  • Treat all callers with courtesy and respect.
  • Facilitate interpersonal communication on a routine basis.
  • Interact with external customers in a courteous and respectful manner.
  • Interact with coworkers and internal customers in a respectful and professional manner.
  • Maintain accountability for actions taken.
  • Perform effectively in the role of preceptor/mentor to new hires and/or students.
  • Process telephone calls in a positive, courteous, and efficient manner.
  • Route calls and messages to appropriate personnel.
  • Maintain cognitive ability to function and concentrate on essential and specialty functions of the job at a high level.
