About The Position

The Patient Service Coordinator ensures patients have been cleared for specialty services office visits. This role serves as the point person for all hospitals, departments, and other healthcare facilities pertaining to questions/concerns on all provider orders/referrals. Provides support to clinical staff in order to facilitate the administrative components of clinical referrals for various services. Requirements

  • High School Diploma
  • Five years experience working with referral processes.
  • Familiarity with EPC EHR system

Responsibilities

  • Gathers pertinent information from insurance carriers, financial counselors, and other ancillary staff to make certain the patient’s financial obligations for services provided.
  • Processes paperwork for new patients efficiently and in accordance with protocols.
  • Verifies insurance coverage and obtains authorizations if needed from insurance carrier.
  • Resolves pre-certification, registration and case-related concerns prior to a patient’s appointment.
  • Tracks and support patients when they obtain services outside the practice e.g. Emergency departments, Hospital and other health care facilities.
  • Monitors and meets PCMH referral processes.
  • Enters referrals and documents communications, actions and other data in an information system.
  • Ensures that all referral requests are properly signed and filed.
  • Care coordination – assists with specialty appointments and obtain needed tests recommended by specialist.
  • Communicates with other team members and referral teams constructively in order to reduce conflicts and enhance the resolution of issues.
  • Be the system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns on orders and referrals.
  • Working collaboratively with other team members, provide care management services for high risk patients.
  • Obtains insurance referrals for patients, getting PA for diagnostic imaging and sending referrals to specialist office for cardiac and preventative screenings.
