About The Position

The Patient Service Coordinator ensures patients have been cleared for specialty services office visits. This role serves as the point person for all hospitals, departments, and other healthcare facilities pertaining to questions/concerns on all provider orders/referrals. The position provides support to clinical staff to facilitate the administrative components of clinical referrals for various services. Responsibilities include gathering pertinent information from insurance carriers, financial counselors, and other ancillary staff to ensure the patient’s financial obligations for services provided are met. The role involves processing paperwork for new patients efficiently and in accordance with protocols, verifying insurance coverage, and obtaining authorizations if needed. It also includes resolving pre-certification, registration, and case-related concerns prior to a patient’s appointment, and tracking/supporting patients when they obtain services outside the practice (e.g., Emergency departments, Hospitals, and other healthcare facilities). The Patient Service Coordinator monitors and meets PCMH referral processes, enters referrals, and documents communications, actions, and other data in an information system, ensuring all referral requests are properly signed and filed. Additionally, the role involves care coordination, assisting with specialty appointments and obtaining needed tests recommended by specialists. It requires communicating with other team members and referral teams constructively to reduce conflicts and enhance issue resolution. The position acts as a system navigator and point of contact for patients and families, providing them direct access for questions and concerns on orders and referrals. Working collaboratively with other team members, the role provides care management services for high-risk patients, obtains insurance referrals, gets prior authorization for diagnostic imaging, and sends referrals to specialist offices for cardiac and preventative screenings.

Requirements

  • Associate’s degree or equivalent knowledge in ambulatory environment.
  • Five years experience working with referral processes.
  • Familiarity with EPC EHR system

Responsibilities
