About The Position

VitalCaring is seeking a Patient Services Coordinator Lead to provide leadership and day-to-day oversight for their centralized patient services coordination team. This role is for an experienced healthcare professional who understands the importance of timely patient care. The Lead will manage a team responsible for scheduling and coordinating patient care, collaborating with branch and clinical leaders to enhance coverage, optimize clinician capacity, and improve the patient and team experience.

Requirements

  • Minimum of 3 years of healthcare experience, preferably within home health, hospice, clinical operations, scheduling, or care coordination.
  • Strong leadership, coaching, and relationship-building skills.
  • Exceptional organization and the ability to manage competing priorities in a fast-paced environment.
  • Strong analytical and problem-solving skills with the ability to turn data into action.
  • A sense of urgency and ownership when patient coverage or scheduling challenges arise.
  • Strong written and verbal communication skills.
  • Experience working with electronic medical records, scheduling platforms, and operational reporting tools.
  • The ability to collaborate effectively with clinical and operational leaders while holding yourself and others accountable for results.
  • A passion for creating efficient processes without losing sight of the patient experience.
  • Proficiency with electronic medical records, scheduling systems, and reporting tools.
  • Current LPN/LVN license in the applicable state (for clinical candidates).
  • Current CPR certification (for clinical candidates).
  • Ability to maintain applicable professional and regulatory standards.
  • Valid driver’s license, reliable transportation, and automobile insurance in accordance with agency requirements (if travel is required).

Nice To Haves

  • Previous experience in a leadership, team lead, training, coaching, or mentoring capacity.
  • Graduate of an accredited Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) program.
  • Previous home health experience and familiarity with clinical scheduling, visit utilization, and clinician productivity.

Responsibilities

  • Provide day-to-day leadership, guidance, and workflow coordination for the centralized Patient Services Coordinator team.
  • Serve as a supervisor, subject matter expert, and escalation resource for complex scheduling and patient coverage needs.
  • Perform Patient Services Coordinator responsibilities while providing leadership and support to the broader team.
  • Monitor staffing coverage, productivity, scheduling effectiveness, and key operational performance measures.
  • Review reports and dashboards to proactively identify scheduling gaps, missed or incomplete visits, and coverage concerns and drive timely resolution.
  • Partner with Branch Directors, Clinical Managers, and field clinicians to improve productivity, scheduling optimization, and patient coverage.
  • Monitor visit utilization and scheduling patterns to help reduce avoidable LUPAs and maximize available clinical resources.
  • Balance workloads and staffing across assigned agencies to support appropriate patient coverage.
  • Provide onboarding, training, coaching, and ongoing development for new and existing Patient Services Coordinators.
  • Conduct quality reviews of scheduling activities and documentation to support accuracy, consistency, and compliance.
  • Identify opportunities to improve workflows, standardize processes, and create more efficient scheduling practices.
  • Facilitate effective communication between centralized scheduling, branch leadership, clinicians, patients, physicians, and members of the interdisciplinary care team.
  • Communicate operational trends, barriers, and opportunities to leadership and help develop solutions.
  • Support QAPI, performance improvement initiatives, and other operational projects related to scheduling and workforce optimization.
  • Foster a culture of accountability, collaboration, urgency, and genuine caring across the team.
