About The Position

We are seeking Patient Intake Coordinators to join a leading healthcare organization supporting patients starting long-term dialysis care. This role is critical to ensuring a smooth, compassionate first experience for patients as they transition into treatment. This is a high-impact, fast-paced role ideal for customer service professionals with healthcare experience who are organized, self-driven, and thrive in collaborative environments.

Requirements

• 1+ year of customer service experience (call center, healthcare office, hospitality, retail, or similar)
• Experience working in a healthcare environment (hospital, clinic, medical office, or health insurer)
• Strong verbal and written communication skills
• High attention to detail and excellent interpersonal skills
• Comfort using computer applications (Outlook, CRM systems, data entry tools)
• High School Diploma or GED

Nice To Haves

• Previous patient intake or care coordination experience
• Dialysis or nephrology experience
• Experience with EMR/EHR systems (Epic, Cerner, Allscripts, etc.)

Responsibilities

• Receive new patient referrals from hospitals and clinics
• Contact patients via outbound calls to coordinate dialysis care and scheduling
• Handle inbound calls from patients following up on scheduling requests
• Manage and track cases independently using Salesforce
• Document outreach efforts, follow up consistently, and close cases accurately
• Work daily within Salesforce (CRM) and Cerner (EHR) systems
• Maintain compliance with patient privacy and HIPAA standards
