About The Position

CHEO is a leading pediatric health organization dedicated to helping children and youth achieve their best lives. CHEO serves over 500,000 children and youth annually from Eastern Ontario, western Quebec, Nunavut, and Northern Ontario. The Patient Service Clerk (PSC) for Home and Community Care works within a multidisciplinary team, supporting Care Coordinators with operational and clerical functions. The role involves providing information and referral services with a high level of client service, ensuring effective communication and follow-up with internal staff, external providers, and the public. The PSC must be available for various shifts, including evenings, holidays, and weekends.

Requirements

  • Secondary School Graduation Diploma (Essential)
  • A minimum of six (6) months of related professional experience in a clinical administrative setting (Essential)
  • Demonstrated knowledge of office procedures including filing and record maintenance and operation of telephone systems and office equipment (Essential)
  • Strong skills related to word processing, and advanced proficiency with Microsoft Office Suite (including Excel, PowerPoint, Word) (Essential)
  • Awareness of and sensitivity to the needs of a culturally diverse population (Essential)
  • Flexible and adaptable with effective problem-solving skills (Essential)
  • Communicates effectively, orally and in writing in both official languages (Essential)
  • Demonstrates excellent interpersonal skills (Essential)
  • Excellent time management and organizational skills, with the demonstrated ability to perform efficiently with attention to detail (Essential)
  • Works effectively in both a team environment and independently as required (Essential)

Nice To Haves

  • One (1) year Community College diploma in Office Administration, Business, Health Administration or equivalent program (Preferred)
  • Two (2) years of related professional experience (Preferred)

Responsibilities

  • Receives and initiates incoming and outgoing telephone calls.
  • Communicates with team members to ensure appropriate information distribution and collection, including handling mail, courier packages, forms, and documents; photocopying and faxing/scanning documents.
  • Maintains an efficient, current, and secure CHRIS client record, including data entry, documenting actions, ensuring file availability, assigning new/transferred client files, uploading documents, accessing client information systems, and processing discharge files.
  • Listens to and documents Care Coordinators’ voice messages, takes and relays messages, refers calls, and arranges for service decreases and equipment collection.
  • Processes requisitions, prepares related documents and form letters, and monitors and orders supplies.
  • Updates client files with new information according to standardized pathways or Care Coordinator direction.
  • Liaises with staff, care providers, families, and pharmacies regarding service confirmations, deliveries, discrepancies, delays, increases, decreases, and discharges, taking appropriate clerical action.
  • Books routine patient home visit appointments and patient/family conferences, prepares agendas, and reserves meeting rooms/books online meetings.
  • Arranges patient transportation by taxi or ambulance as authorized.
  • Assists with projects and initiatives related to job responsibilities, participates in orientation and committees, and performs other related duties as requested.
  • Acts as a resource for CHEO employees, service providers, school personnel, and the community.
  • Attends and actively participates in team and general staff meetings.
  • Assumes responsibility for maintaining professional knowledge and updated skills.
  • Performs work in accordance with the Occupational Health and Safety Act and Regulations and CHEO policies.
  • Performs other related duties as assigned by supervisor.
