The Patient Safety Coordinator supports the organization's commitment to high reliability, patient safety, and continuous performance improvement. This position coordinates and facilitates patient safety investigations, Root Cause Analyses (RCAs), Apparent Cause Analyses (ACAs), and Intensive Case Reviews (ICRs). Develops and monitors corrective action plans; ensures compliance with regulatory reporting requirements; and partners with operational and clinical leaders to reduce risk and improve patient outcomes. The coordinator promotes a Just Culture approach focused on systems improvement, organizational learning, and prevention of future harm. Conducts medical record reviews, data collection, timeline development, and staff interviews to facilitate the identification of event contributing factors, system vulnerabilities, and opportunities for improvement while ensuring investigations are completed within established organizational and regulatory timeframes. Maintain patient safety investigation files and support documentation. Facilitates multidisciplinary meetings to develop sustainable corrective actions that address identified root causes. Assist operational leaders in developing measurable action plans and monitoring strategies. Utilize structured methodologies to define responsible, accountable, consulted, and informed stakeholders (RACI). Presents action plans and investigation findings to Patient Safety, Quality, and Leadership Committees as appropriate. Coordinate Department of Health, Patient Safety Organization (PSO), and other regulatory patient safety reporting requirements. Maintain confidentiality and protections associated with Patient Safety Work Product (PSWP). Support the Patient Safety Committee, Safety Coaches, and organizational safety initiatives. Drives and supports high reliability initiatives.
