About The Position

The Patient Safety Coordinator supports the organization's commitment to high reliability, patient safety, and continuous performance improvement. This position coordinates and facilitates patient safety investigations, Root Cause Analyses (RCAs), Apparent Cause Analyses (ACAs), and Intensive Case Reviews (ICRs). The role involves developing and monitoring corrective action plans, ensuring compliance with regulatory reporting requirements, and partnering with operational and clinical leaders to reduce risk and improve patient outcomes. The coordinator promotes a Just Culture approach focused on systems improvement, organizational learning, and prevention of future harm. This involves conducting medical record reviews, data collection, timeline development, and staff interviews to identify event contributing factors, system vulnerabilities, and opportunities for improvement, ensuring investigations are completed within established organizational and regulatory timeframes. The position also requires maintaining patient safety investigation files and support documentation, facilitating multidisciplinary meetings to develop sustainable corrective actions, assisting operational leaders in developing measurable action plans and monitoring strategies, utilizing structured methodologies like RACI, and presenting action plans and investigation findings to relevant committees. Additionally, the role coordinates regulatory patient safety reporting requirements and maintains confidentiality of Patient Safety Work Product (PSWP), while supporting the Patient Safety Committee, Safety Coaches, and organizational safety initiatives, driving high reliability initiatives.

Requirements

  • Healthcare experience or clinical experience such as nursing or other licensed professional.
  • Ability to navigate charts and identify opportunities.
  • Valid Driver's License.

  • Assists operational leaders in developing measurable action plans and monitoring strategies.
  • Utilizes structured methodologies to define responsible, accountable, consulted, and informed stakeholders (RACI).
  • Presents action plans and investigation findings to Patient Safety, Quality, and Leadership Committees as appropriate.
  • Coordinates Department of Health, Patient Safety Organization (PSO), and other regulatory patient safety reporting requirements.
  • Maintains confidentiality and protections associated with Patient Safety Work Product (PSWP).
  • Supports the Patient Safety Committee, Safety Coaches, and organizational safety initiatives.
  • Drives and supports high reliability initiatives.
