As an integral member of the clinical team, the Patient Resource Coordinator plays a vital role in ensuring the smooth and efficient daily operations of the Outpatient Transplant clinic. The principal responsibilities include rooming patients in examination rooms, measuring and recording height, weight and vital signs, conducting basic nursing assessments, collecting lab specimens and swabs, performing wound care as needed, educating and instructing patients and family members on relevant health topics, and reviewing and documenting patient medications (medication reconciliation). This role also involves performing cross-functional and/or other duties consistent with the job classification, as assigned or requested. The coordinator ensures the day-to-day transplant clinics are running efficiently while providing quality patient-centered care across various clinics. They triage patients in clinic, prioritizing clinic needs in collaboration with medical staff and the operations manager/PCC. The role applies knowledge of the nursing process, as it relates to scope of practice, to achieve identified patient goals. The coordinator identifies and recognizes abnormal or unusual patient responses and consults with the nurse coordinator or MRP in clinic. Practice is ensured to be consistent with professional standards and the ethical framework as outlined by the College of Nurses of Ontario and UHN policy and procedure. The role demonstrates commitment to professional life-long learning by participating in continuing education activities to meet the needs of the patient population served and the standards of the profession. This includes performing reflective practice, analyzing and evaluating nursing knowledge and modifying practice accordingly to improve the quality of nursing and patient care. The coordinator liaises with relevant parties, such as physicians, nurse coordinators, and other allied staff as needed, and communicates/coordinates/collaborates with Nurse Managers on opportunities or issues in the clinic setting.
