About The Position
As a Patient Resource Coordinator, you’ll help patients overcome social, financial, and environmental barriers so they can access and continue care. You’ll coordinate enabling services, connect individuals and families to internal and community supports, and provide limited clinical assistance within the CNA scope to support efficient visits. Requirements
- Current North Dakota CNA certification
- Certified Application Counselor (CAC) certification preferred or ability to obtain within 90 days
- Willingness to travel between locations; occasional evening/weekend availability as needed
Responsibilities
- Linking patients to community resources, financial assistance, and insurance coverage options
- Coordinating with medical, dental, behavioral health, care management, and patient services teams to improve continuity of care
- Supporting patient navigation and reducing obstacles to treatment
- Assisting with patient flow and basic clinical tasks within CNA scope
