Patient Navigator - Belvidere and Stanhope, NJ
About The Position
Crossroads Treatment Centers is seeking a Patient Navigator to join their team. Since 2005, Crossroads has been a leader in treating patients with opioid use disorder, providing accessible, high-quality, evidence-based medication-assisted treatment (MAT). They are dedicated to combating the opioid epidemic and helping individuals achieve long-term recovery. Crossroads operates outpatient, office-based clinics across multiple states and is committed to improving treatment access for a significant number of patients. Requirements
- Must hold one of the following credentials/experiences: RN, LPN, or BSW
- Bachelor’s Degree WITH at least 2 years of lived addiction experience.
- Associate's degree or CMA WITH 4 years of lived addiction experience.
Responsibilities
- Greeting patients and answering clinic phones.
- Assisting patients in understanding their treatment options and resources.
- Serving as an essential link between patients and all other care providers.
- Facilitating appointments for consults and support services, including social workers and physicians.
- Coordinating multidisciplinary planning conferences, developing patient summaries for the care team, and documenting recommendations using standardized care protocols.
- Following patients throughout the care continuum, including inpatient admissions, and collaborating with inpatient care management.
- Working with staff to collect data, track outcomes, and support strategic planning.
- Conducting outreach efforts to establish and maintain positive working relationships with physicians, office staff, nurses, and social services.
- Assisting the Center in stocking supplies and supporting the Patient Ambassador team.
- Performing other duties as assigned.
